I think he’s likely in a vegetative state more than brain dead, as whatever magic affecting Tokyo (and by proxy, keeping him alive,) would have likely been cut off after what happened with Michael. Yes, the Old Ones would have magic, but I doubt it’d keep him alive long enough.

He's not exactly "awake". The one that's awake is an avatar of him which means the Azathoth sitting on the throne is the real Azathoth, that looks like he's in a coma but I think he's dreaming and controlling his avatar while at it.
